About the Role
Heartland Hospice, part of ProMedica, is seeking a compassionate Registered Nurse to provide hospice care in home and facility settings. The RN delivers skilled nursing, pain and symptom management, and emotional support to patients with life-limiting illnesses and their families. Responsibilities include comprehensive assessments, individualized care planning, coordination with the interdisciplinary team, patient and family education, and timely documentation. The RN participates in on-call rotation, collaborates closely with physicians and social workers, and upholds the organization’s mission of dignity, comfort, and respect at end of life.
Responsibilities
- Provide skilled nursing and care coordination for hospice patients
- Collaborate with interdisciplinary team and provide education to patients/families
- Participate in on-call rotation and documentation practices
Qualifications
- Registered Nursing license
- Hospice or palliative care experience preferred
- BLS/ACLS certifications
