About the Role
Oak Street Health seeks a part-time Primary Care Physician to deliver high-quality, preventative care to adult Medicare patients in underserved communities. In this patient-centered, team-based role, you will manage chronic conditions, perform wellness visits, and coordinate care with nurses, social workers, and other clinicians. Responsibilities include thorough clinical assessments, evidence-based treatment, and clear documentation in the EHR. You’ll help drive quality improvement, support population health initiatives, and contribute to an inclusive, collaborative culture focused on equity and community impact.
Responsibilities
- Provide primary care to adult Medicare patients in underserved communities

- Coordinate care with nurses and clinicians
Qualifications
- MD or DO license in good standing
- Board-certified/eligible in primary care or internal medicine
- Active US medical license
