Outpatient Vascular Nurse Practitioner
About the Role
Community Care Physicians is seeking an Outpatient Vascular Nurse Practitioner or Physician Assistant to join its physician-owned multispecialty group in New York’s Capital Region. Provide patient-centered outpatient vascular care, assess and manage patients, coordinate diagnostic evaluation, develop treatment plans, and educate patients on follow-up and wellness. Collaborate with physicians and care teams while documenting care and supporting quality improvement. Candidates must hold an active New York NP or PA license and relevant board certification; vascular experience is preferred.
