Outpatient Vascular Nurse Practitioner
About the Role
Community Care Physicians is seeking an Outpatient Vascular Nurse Practitioner or Physician Assistant to provide patient-centered care in New York’s Capital Region. In this role, you will assess patients with vascular concerns, obtain histories, perform examinations, develop care plans, document clinical findings, and educate patients about treatment and follow-up. Work collaboratively with physicians and care teams in a supportive, physician-owned multispecialty group focused on quality improvement and lasting patient relationships. This opportunity offers access to strong support teams and professional growth across outpatient specialties and care centers. Current New York licensure and national certification are required.
