About the Role
VNS Health is seeking a compassionate, skilled per diem Occupational Therapist to provide home- and community-based care in New York City. You will assess patients’ functional abilities, develop individualized treatment plans, and deliver interventions that help people improve independence and safely manage daily activities. Partner with patients, families, caregivers, and an interdisciplinary team to set goals, recommend adaptive equipment, support home safety, and document progress. Responsibilities
- Assess patients' functional abilities and develop treatment plans

Qualifications
- Licensed Occupational Therapist in NY
- Per diem/contract position
- Experience in home and community-based care preferred
