About the Role
Signify Health is seeking an In-Home Physician to perform comprehensive health evaluations for vulnerable and chronically ill patients in their homes. You will conduct risk assessments, medication and safety reviews, close care gaps, and document findings accurately in digital tools to support value-based care. Partner with care coordinators and health plans to create actionable care plans and connect patients with community resources. You’ll practice with flexible scheduling, light procedural demands, and strong clinical autonomy, supported by a collaborative, data-driven health tech culture focused on improving outcomes.

Qualifications
- Medical license in home-based practice
- Board-certified physician preferred
- Experience with value-based care or in-home primary care is a plus
