About the Role

Coordinate individualized hospice care for patients in their homes across the Woburn, Waltham, and Watertown areas. Assess patients, collaborate with the interdisciplinary care team, develop care plans, educate patients and caregivers, document progress, and support pain and symptom management. Current state RN licensure, CPR certification, and at least one year of nursing experience are required.
