About the Role
Community Care Physicians is seeking a Family Medicine NP/PA to join our outpatient practice in Slingerlands. In this role, you’ll provide comprehensive primary care for patients of all ages, including acute visits, chronic disease management, preventive care, and wellness counseling. You’ll work within a collaborative, physician-owned group that emphasizes patient-centered care, quality improvement, and work–life balance. Responsibilities

Qualifications
