Case Coordinator
About the Role
St. Vincent’s Medical Center, part of Hartford HealthCare, is seeking a compassionate Case Coordinator to support patients and care teams in Bridgeport, Connecticut. In this patient-centered role, you will coordinate referrals, appointments, and transitions across clinical, behavioral health, and community services. You’ll communicate with patients, families, providers, and community partners; document case activity; identify barriers to care; and help connect people with appropriate resources. Work collaboratively with multidisciplinary teams to promote safe, timely outcomes and improve community health.
